Instructions:

1. Choose the format you wish to use (MS-Word Doc, Rich Text Format, PDF).
2. Right click on the appropriate link below.
3. Select "Save Target As" and save the file to your computer.
4. If you download the MS-Word Doc or the Rich Text Format, then you can type in your information with your word processor and save it.
   If you download the PDF file, then print it and fill it out by hand.
5. Print the application and send it with your check to the address on the form.
